Our primary care clinics take the time to get to know you and your family. After all, we’re neighbors! Your health is important and we want to be your partners in this effort with our seven primary care clinics in the northern Illinois and southern Wisconsin area. Our clinics offer comprehensive healthcare for all ages and include:

We Have Specialized Care In Our Outreach Centers

Physicians in our outreach centers have access to specialized care when needed and will be your advocate to refer you, if necessary. After office hours and on weekends, calls to the outreach centers will be forwarded to an on-call physician. As our patient, this is our guarantee that you have access to medical assistance 24 hours a day, seven days a week.
