In order to utilize this service, you must provide all requested information; otherwise, the refill cannot be completed. Please be prepared to provide the following information:
- Your Name and Phone Number
- Patient's Name
- Patient's Date of Birth
- Patient's Telephone Number
- Name of Medication
- Dosage
- Directions Printed on the Label
- Name and Phone Number for the Pharmacy You Wish to Use
Refill requests will be completed 48 hours or two business days from the time they are left.
