Patient and Family Services

Patient and Family Services provides assistance with a variety of needs, both in the hospital and planning for care at home following hospitalization. Staff can assist in arranging for home health care, out-of-home placement (such as nursing home or Assisted Living), Meals on Wheels, durable medical equipment, transportation, referrals to community resources and other at-home needs. While the patient is in the hospital, staff offer support to the patient and family in dealing with illnesses and hospitalization. Staff can also assist with Advance Directives.
