You're invited to attend this two hour class on Physical Activity/Blood Glucose Monitoring as part of the Diabetes Education Self-Management Program.
Classes are held from 4 to 6 pm at Beloit Memorial Hospital. A minimum of two participants are needed to offer the class.
Please check in with the hospital front desk on arrival so staff can escort you to the meeting site.
Registration is required and most insurance is accepted.
