• Dyspnea/Increased dependency on O2

• Unmanageable pain

• Cognitive decline

• Advancing Dysphagia

• Skin breakdown

• Dramatic weight loss

• Recurring Infections

• Recurring falls

• Recurring hospital/ER visits

• Increased need for assistance with ADLs
