Eligibility Criteria

1. Symptomatic on optimal diuretic and/or vasodilator therapy (ACE-1, ARB)
2. Significant symptoms of recurrent CHF at rest and NYHA Class IV (any activity causes discomfort)
3. Not a candidate for surgical procedures or patients decline these procedures, has a medical reason for refusing meds e.g., low BP, kidney disease, etc.

Supporting Documentation

1. EF <20% (for left heart failure)
2. Diastolic dysfunction and degree
3. Treatment-resistant symptomatic supraventricular or ventricular arrhythmias
4. History of cardiac arrest or resuscitation
5. History of unexplained syncope
6. Brain embolism of cardiac origin
7. HIV disease
