Eligibility Criteria
- Disabling dyspnea at rest, poorly or unresponsive to bronchodilators resulting increased functional capacity (bed to chair existence, fatigue, cough)
- Progression of end stage lung disease, shown by increasing visits to ER or hospitalizations for respiratory infections and/or respiratory failure or increasing physician home visits or contacts before initial certification
Supporting Documentation
- FEVI <30% after bronchodilator
- Serial decrease of FEVI > 40 ml/yr
- Hypoxia on room air (O2 sat less than or equal to 88% - may be from recent hospitalization) or Hypercapnia (pCO2 > or equal to 50 mmHg within last 3 months)
- Right hear failure due to lung disease (cor pulmonale) (not from left heart disease or valvulopathy)
- Unintentional progressive weight loss of 10% over last 6 months
- Resting tachycardia of >100/min
