Beloit Health System is seeking a progressive and experienced Director of Nursing for our Transitional Care Services. This position will have oversight of the System’s Beloit Regional Hospice, At-Home Healthcare, Assisted Livings and Palliative Care teams. Transitional Care Services provide services to outpatients that transcend a patient’s adult needs across the lifespan.

The Director will have oversight of two assisted living facilities: NorthPointe Terrace, a 24-apartment facility located on our NorthPointe Campus in Rockton, Il, and Riverside Terrace, a 45-apartment facility in Beloit, Wisconsin. Beloit Regional Hospice focuses on comfort and quality of life to make each moment of a patient’s remaining life as full and as comfortable as possible. The care team consists of a physician, nurse, CNAs, social workers, chaplains, and volunteers. At-Home Healthcare provides quality home health services in the comfort of the patient’s home for those who have just returned home from the hospital or nursing home, have an illness, chronic disease or are recovering from an accident or surgery, or are in need of physical, occupational, or speech therapy. For the sixth consecutive year, Beloit Health System At-Home Healthcare has been awarded the Patient Satisfaction Award of Distinction by WellSky. Palliative Care is specialized medical care for people with serious illness. Care can begin as soon as diagnosis and is focused on providing relief from the symptoms, pain, and stress of a chronic or lasting acute illness.

Reporting to the Chief Nursing Officer, the Director will provide overall leadership, daily management, and direction for the service line including but not limited to providing strategic direction and leadership for staffing systems and strategy within the department. The Director will serve as a resource to manage daily operations of clinical staffing with the expectation of long-term financial savings through enhanced utilization, scheduling, and deployment of staff. They will provide coordination of staff services across the organization with the intent of creating improved responsiveness to patient needs, and provide overall strategic project management, facilitation, and ongoing maintenance of a centralized staff scheduling/deployment system utilizing a software platform to manage staff.

The Director of Nursing, Transitional Care Services will collaborate with the leadership of Beloit Health System’s multidisciplinary teams to facilitate an excellent patient experience and to systematically improve assessment, quality, and patient experience. The individual selected for this critical role regularly interacts with the senior leadership of Beloit Health System, as well as colleagues throughout the Health System and community.

This position requires a Bachelor's Degree in Nursing, with a MSN preferred, and a current IL and WI RN License. Progressive leadership experience is preferred.
